91 Automatic jumps out of gear

91 Automatic jumps out of gear
Have a 91 EX, with over 200k, that will occasionally jump out of D4 into
neutral. Also, you can move the shifter between the gear positions without
pushing the shifter button.
Is this a problem with the transmission or with the shift lever mechanism?

n, d4 & d3 all shift without the lock. if you jump from d4 to n, check
the cable adjustment or tell your passenger to stop doing what they're
doing.

the cable adjustment or tell your passenger to stop doing what they're
